          What is Semaglutide?

           

          “An early study of 2,000 obese adults compared people using semaglutide plus a diet and exercise program with people who made the same lifestyle changes without semaglutide. After 68 weeks, half of the participants using semaglutide lost 15% of their body weight, and nearly a third lost 20%. Participants who incorporated only lifestyle changes lost about 2.4% of their weight.” – UCLA Health

          Semaglutide, found in Wegovy and Ozempic, is a medication designed to address both type 2 diabetes and obesity. It falls within the category of medications known as GLP-1 receptor agonists. Semaglutide functions by imitating the actions of a naturally occurring hormone called glucagon-like peptide-1 (GLP-1). When taken as directed, it assists in regulating blood sugar levels by increasing insulin production and reducing the release of glucose from the liver. Typically, this medication is used in conjunction with dietary and exercise measures to enhance glycemic control in individuals with type 2 diabetes. Additionally, it may be prescribed for other medical conditions as determined by a healthcare professional.

          Semaglutide shares a remarkable 94% similarity with the naturally occurring human hormone, glucagon-like peptide 1 (GLP-1). This similarity allows it to reduce blood sugar levels in a manner dependent on glucose, primarily by enhancing insulin secretion. GLP-1 also acts as a natural appetite regulator, reducing food cravings while increasing feelings of fullness and satisfaction. Moreover, it slows down gastric emptying and curbs appetite, often leading to weight loss in many patients. As a result, healthcare providers have been using semaglutide for over 15 years to manage Type 2 diabetes. While there are various anti-obesity medications that help control appetite and promote weight loss, semaglutide stands out for its exceptional effectiveness in this regard.

          After a clinical trial to study semaglutide weight loss, Dr. John Wilding at the Endocrine Society at the University of Liverpool said,

          Tirzepatide acts on both GIP and GLP-1 receptors, leading to similar effects as semaglutide but potentially with a stronger blood sugar-lowering effect and greater weight loss.

          Dosing:

          Semaglutide: Semaglutide is available in various formulations, including once-weekly injections and a daily oral tablet.

          Tirzepatide: Tirzepatide is typically administered as a once-weekly injection. It has a more extended duration of action compared to some other GLP-1 receptor agonists.

          Weight Loss:

          Both semaglutide and tirzepatide have been associated with significant weight loss in clinical trials. However, tirzepatide, being a dual receptor agonist, may have a more pronounced effect on weight loss compared to semaglutide.

          What are the Side Effects?

           

          Tirzepatide & Semaglutide, like many medications, can have side effects. Common side effects of both Tirzepatide & semaglutide may include:

          Nausea: Some people may experience nausea when first taking semaglutide. This side effect often improves over time as your body gets used to the medication.

          Vomiting: Vomiting can occur, especially when nausea is present. It’s important to stay hydrated if you experience vomiting.

          Diarrhea: Diarrhea may occur as a side effect. It’s important to monitor this and seek medical advice if it becomes severe.

          Abdominal Pain: Some individuals may experience abdominal discomfort or pain.

          Decreased Appetite: Semaglutide & Tirzepatide can lead to a decreased appetite in some people, which may contribute to weight loss.

          Hypoglycemia (Low Blood Sugar): While less common than with some other diabetes medications, hypoglycemia can occur, especially if semaglutide is used in combination with other diabetes drugs that can lower blood sugar.

          Injection Site Reactions: Redness, swelling, or itching at the injection site can occur.

          Pancreatitis: Rarely, pancreatitis (inflammation of the pancreas) has been reported as a side effect. Seek medical attention if you experience severe abdominal pain.

          Thyroid Tumors: In clinical trials with rodents, semaglutide has been associated with thyroid tumors. However, it’s unclear if this risk applies to humans.

          The side effects of semaglutide and Tirzepatide are typically mild — especially when compared to the complications associated with obesity. Gastrointestinal issues, such as diarrhea, constipation and gassiness are the most common complaints among people just starting out. But you may be able to reduce the side effects by beginning on a lower dose and then slowly increasing the amount you take.  It is important to discuss the possible side effects of any medication with your healthcare provider. 

          Side Effects & Symptom  Treatment Options:

           

          Recommendations from our Registered Nurses and Medical Director

          Nausea:

          Nausea is a common initial experience when commencing Semaglutide or Tirzepatide treatment, attributed to the delayed gastric emptying and digestive processes.

          Our recommended approaches to alleviate this symptom include:

          • Zofran ODT, taken as needed every 6 hours.
          • Compazine, as needed twice daily.
          • Small, frequent snacks throughout the day, preferably rich in protein.
          • Pre-dose intravenous (IV) fluid infusion.

          Constipation:

          Constipation may arise during Semaglutide or Tirzepatide due to the decelerated gastric emptying and digestion. Our suggested measures for symptom relief are as follows:

          • Over-the-counter stool softeners, taken twice daily.
          • Ensuring proper hydration with a minimum intake of 32 ounces of water daily.
          • Maintaining physical activity – do not become immobile as this worsens symptoms.
          • Miralax may be considered for severe constipation as deemed necessary.

          Fatigue:

          Some individuals may experience mild fatigue during the initial 1-3 weeks of Semaglutide or Tirzepatide usage. 

          • To address this mild fatigue, prioritizing adequate sleep and engaging in regular physical activity can be beneficial.
          • It’s important to note that this fatigue typically diminishes and often dissipates entirely by week 3-4 of treatment.

           

          IF YOU ARE HAVING SURGERY WHERE YOU WILL BE PUT TO SLEEP, IT IS HIGHLY RECOMMENDED THAT YOU STOP YOUR SEMAGLUTIDE OR TIRZEPATIDE AT LEAST 7 – 10 DAYS PRIOR – DUE TO THE SLOWING OF THE GASTRIC EMPTYING

          Semaglutide:

          All new semaglutide patients are required to follow our dosing protocol. Patients begin with a dose of .25 once weekly for 4 weeks. The continued dosing schedule is as follows:

          Weeks 1 – 4: 0.25mg

          Weeks 5 – 8: 0.50mg

          Weeks 9 – 12: 1.0mg

          Weeks 13 – 16: 1.5mg

          Weeks 17 and onward: 2.0  or 2.5mg – maximum dose

           

          Tirzepatide:

          All new tirzepatide patients are required to follow our dosing protocol. Patients begin with a dose of 2.5mg once weekly for 4 weeks. The continued dosing schedule is as follows:

          Weeks 1 – 4: 2.5mg

          Weeks 5 – 8: 5.0mg

          Weeks 9 – 12: 7.5mg

          Weeks 13 – 16: 10mg

          Weeks 17 – 21: 12.5mg

          Weeks 22 and onward: 15mg – maximum dose

          Please be advised that irrespective of prior dosage experiences, all patients are obligated to adhere to our prescribed dosing regimen unless expressly authorized by our Medical Director, Dr. Hruby.

          How to Get Started:

           

          All new patients will be required to fill out a brief health history and Client Information Form to be sure that you are a suitable candidate for either medication. 

          Step one: Set an appointment, either online at ivclub.com or by calling 605-271-0715

          Your initial appointment will be an opportunity to speak with our registered nurses about whether your health history makes you the right fit, and answer any questions you may have before you begin. This process takes about 15 minutes in total. Your RN will complete an exam with vital signs and a health history review. This exam is conducted before clients start their first dose of Semaglutide or Tirzepatide – this is to ensure the medication is an appropriate elective option per our standards and our protocol authorized by Dr. Hruby.

          Step Two: Complete your Health History and Client Information. 

          These documents will be sent to you automatically upon scheduling your initial appointment. You have the option to fill them out electronically or complete them in person at our office. It is important to note that your Health History Form determines your eligibility to commence these medications. For efficiency, we recommend completing these forms prior to your scheduled visit. If you have any questions about your medical history, you must speak with your Primary Care Provider for guidance.

          Step Three: Ongoing Weekly Appointments. 

          Subsequent appointments are expected to require no more than 5 minutes of your time. For added convenience, we offer the option to securely store your payment details on your account, streamlining the checkout process.

           It is essential to emphasize that this regimen adheres to a weekly schedule, involving incremental dosage adjustments over time.
